There are several Pony Club forms
that may have to be completed and turned in multiple times throughout
the course of the year. The two forms that are always required at
official Pony Club events are the Medical Release and the Activity
Release forms.
"Original" signatures and
dates are Required on each form when they are submitted,
so here's the trick:

Fill out the two forms at the
beginning of the year. Fill in all required information EXCEPT:
(You may want to also leave blank
the "Current Rating" if that is likely to change during
the course of the year.)

Make a number of copies (8 or 10)
of each form and keep them on file.
Then, when it's time to submit a new
set of forms for an event, just pull out the completed copies and fill
in the specific event info and sign and date them.
NOTE: The 2nd page of
the Medical Release form does NOT need to be filled out or signed by a
physician unless there is a medical problem. That whole second page
can be ignored if the child has no health concerns.
